II. List of Requirements:

 Practicum 42 , please refer to page 74 (Appendix T. Pract42 Requirements Checklist)-note: please arrange the documents accordingly.

Practicum 41, please refer to page 34 (Appendix I. Pract41 Requirements Checklist)-note: please arrange the documents accordingly.

Please include signed/accomplished MOA(Memorandum of Agreement.

III. Submit all your Midterm requirements in Brown Envelope (Long) and label your envelope(pls write neatly) as follows:

IV.  Your Nameplate and Practicum Uniform (Type A) will be checked and please wear it during submission.

V. Submitting beyond the deadline will not be accepted and therefore Midterm and Final Grades will automatically be DR.
