To all  Practicum42 students - Summer 2019

i.) Deadline for submission of  Practicum Requirements-Midterm will be on  May 7, 2019 (Tuesday). (pls refer to your Practicum Manual- list of practicum requirements-Page 22/34)


Also, please include the following :

1. filled-out Memorandum of Agreement (MOA)and/or contracts

2. filled-out Practicum Journal /Diary (date: from start  of your OJT until May 6, 2019)  

Submit the documents together with your Practicum Manual  in Long Brown Envelope with label as follows:

        Edpcode and Subject :

        Adviser:

ii) Submission of Final Requirements: May 15, 2019(Wed)


Note: 
Please be reminded that submitting beyond the deadline will not be accepted and therefore Midterm and Final Grades will automatically be DR.
