I. Schedule for submission of Practicum - Midterm Requirements as follows :

Practicum 42 with EDP CODE :

Practicum 41 with EDP CODE:

II. List of Requirements:

 Practicum 42 , please refer to page 74 (Appendix T. Pract42 Requirements Checklist)

Practicum 41, please refer to page 34 (Appendix I. Pract41 Requirements Checklist)

Note: Those who have not submitted the copies of signed/accomplished SIC /MOA and psychological exam result, please include it.

 III. Submit all your Midterm requirements in Brown Envelope (Long) and label your envelope as follows:

IV.  Your Nameplate and Practicum Uniform (Type A) will be checked and please wear it during submission (if any).

V. Submitting beyond the deadline will not be accepted and therefore Midterm and Final Grades will automatically be DR.
