Practicum Initial Requirements
Dear Practicum students(First Sem 2021-2022):
Practicum Initial Requirements
1. To request for Printing of Recommendation letter (Appendix H – ) , attached Application letter (with signature) and Resume
2. Once the company specified on the request is approved, a Recommendation Letter will be released to student together with the confirmation slip (blank)
3. Student will submit a filled-out Confirmation letter/slip (Appendix J ) signed by HTE to CCS Office
4. Once the filled-out confirmation letter is submitted to CCS Office , MOA and Student Internship Contract(SIC) will be released to student for processing.
