Please take note of the following:

I. Schedule for submission of Practicum - Midterm Requirements as follows :

Practicum 42 with EDP CODE : 

01214(adviser:Dr. Niñal) and 01263 (adviser: Ms Amores)- on August 8, 2018(Wed); anytime during your vacant time; at CCS office

Practicum 41 with EDP CODE: 

01131(Advisers: Ms. Amores/Ms. Carumba) - on August 10, 2018(Friday),   @CCS office

01164(Advisers: Dr. Niñal/Ms. Carumba) - on August 13, 2018(Monday),   @ CCS office

01107(Advisers: Ms. Ybañez/Ms. Carumba) - - on August 14, 2018(Tuesday) , @ CCS office

II. List of Requirements:

 Practicum 42 , please refer to page 74 (Appendix T. Pract42 Requirements Checklist)

Practicum 41, please refer to page 34 (Appendix I. Pract41 Requirements Checklist)

Please include signed/accomplished MOA(Memorandum of Agreement.

III. Submit all your Midterm requirements in Brown Envelope (Long) and label your envelope as follows:

IV.  Your Nameplate and Practicum Uniform (Type A) will be checked and please wear it during submission.

V. Submitting beyond the deadline will not be accepted and therefore Midterm and Final Grades will automatically be DR.
