What: New MOA and Contract Templates for Student Internship Program (SIPP)

When: Effective 2nd Semester, S.Y. 2018-2019

How:

• There are two templates to be followed namely: 1)MOA between the HEI and HTE and 2)Student Internship Contract
• These templates can be downloaded from this link: Download here.
• The SIPP MOA must be accomplished for each HTE, distinctly (This will be facilitated by the HEI through the Practicum Teachers, SIPP Coordinator and the Dean)
• Notarized copies of the SIPP MOA must be kept by or given to:
  • The Legal Office
  • The Dean
  • The SIPP Coordinator
• The Student Internship Contract must be accomplished by each Student-Intern by:
  • downloading the file, 
  • filling out the information thoroughly, 
  • printing it out, 
  • having the original copy signed by all signatories
  • having the signed contract notarized
  • reproducing the notarized contract in 5 copies
• Notarized copies of the Student Internship Contract must be kept by or given to:
  • the HTE Trainer
  • the SIPP Coordinator
  • the Dean
  • the Student Trainer
  • the SI compilation
• Accomplished Student Internship Contracts must be submitted on or before December 19, 2018.
