Freshmen requirements - Original Copy of Report Card, Original Copy of Good Moral Character Certificate and Photocopy of NSO/PSA Birth Certificate

Transferees requirements -Certificate of Transfer Credentials, TOR for Evaluation, Good Moral Certificate and Photocopy of NSO/PSA Birth Certificate
